Application Parts I &II

Part 1 covers the student's/parent's information i.e. address, phone numbers, etc.

Part 2 is information we ask of the student to help us better meet the needs of the student.

English Recommendation

This form is to be filled out by your current English teacher and  mailed to St. Francis Xavier High School.

Medical Release & Parental Permission

The Medical Release form gives us important information about your child's medical condition and consent for us to obtain emergency medical treatment if necessary.

The Parental Permission form gives your child permission to go on any trips sponsored by our school.

Math Recommendation

This form is to be filled out by your current Math teacher and  mailed to St. Francis Xavier High School.

Physical & Pre-Participation

If your child participates in any athletics, both forms will need to be filled out.  The physical has to be completed by a doctor. Both forms must be updated annually.

SCISA Sports Agreement and Warning of Inherent Risk

SCISA Sports Agreement - Parent's/Student's code of conduct at sporting events.

Warninng of Inherent Risk -  Student's/Parents understanding that there are risks when participating in athletics and their legal rights.

Medication Procedures

This form gives us permission to dispense medication and the procedures for bringing prescription medications in for students.

Internet Use Agreement

Student's code of conduct for use of the Internet on school property.

Transportation Form

Permission to transport your child to sporting events and to transport your child from church to school on Mass days.

Returning Student's Application  Form

Students who are returning to St. Francis Xavier need to fill out this application form instead of Application Parts 1 & II
