St. Francis Xavier High School: Admissions Forms

Application Registation for the 2018-2019 school year. St. Francis will merge with St. Anne and St. Jude Catholic School to become a K-12th grade.	English Recommendation This form is to be filled out by your current English teacher and mailed to St. Francis Xavier High School.
Medical Release & Parental Permission The Medical Release form gives us important information about your child's medical condition and consent for us to obtain emergency medical treatment if necessary. The Parental Permission form gives your child permission to go on any trips sponsored by our school.	Math Recommendation This form is to be filled out by your current Math teacher and mailed to St. Francis Xavier High School.
Physical Examination Form Pre-Participation History & Health Assessment Form If your child participates in any athletics, both forms will need to be filled out. The physical has to be completed by a doctor. Both forms must be updated annually.	SCISA Sports Agreement SCISA Sports Agreement - Parent's/Student's code of conduct at sporting events.
Medication Procedures This form gives us permission to dispense medication and the procedures for bringing prescription medications in for students.	Internet Use Agreement Student's code of conduct for use of the Internet on school property.
SCISA Inherent Risk Warninng of Inherent Risk - Student's/Parents understanding that there are risks when participating in athletics and their legal rights.
